If you are interested in opening a wholesale account please fill out this Customer Application and send it back to us along with a copy of your resale certificate or business license. Please send us the forms via email or fax (813-982-2242). Once we receive your information we will send you our current price list and availability. We can ship door to door with FedEx or USPS. We also use air cargo with arilines such as SouthWest, Delta, American Airlines, and United Air Cargo.
